Care Services
Riverside Health and Rehab is a 160 bed skilled nursing facility in Charleston, South Carolina. Centrally located, Riverside offers easy access to Charleston hospitals. It houses a specialty CardioPulmonary Pavilion, as well as provides short-term rehabilitation, skilled nursing care, long-term care, hospice care, and respite care.
Accepting Medicare and most major insurances, Riverside is able to provide care for anyone 18 years and older. Along with the experienced nursing staff Riverside has highly trained therapists that provide Physical Therapy, Speech Therapy, Occupational Therapy, and Respiratory Care. Riverside also provides Wound Care, Tracheotomy Care, IV therapy, and Tube Feeding Care.
Patient-centered care is at the heart of what we do at Riverside Health and Rehab. We work together, and listen to your needs to obtain a common goal of improving functioning so that you can return to a full, productive life.
Long-term Care
Long-term care assists those who may benefit from 24-hour nursing availability and assistance with meals, medication management, and other activities of daily living.

Short-term Care
Individualized therapy and treatment is provided by skilled clinicians in our motivational therapy gym. Our facility is designed with the very latest therapy assessment, monitoring, and treatment modalities. Our rehabilitation strategy is tailored to the individual. We do this by simulating an environment that resembles the home environment as much as possible.
Rehabilitation
A team of therapists, under the direction of an independent physiatrist and the support of certified therapy assistants, develop a plan of care for each patient. We offer Physical, Occupational and Speech therapies.
Physical Therapists will evaluate:
- Joint Range of motion
- Muscle Strength
- Ability to transfer out of bed as well as other surfaces
- Ability to Walk
- Balance
- Prior level of function
Based on the evaluation, a care plan will be developed and may focus on:
- Teaching strengthening and range of motion exercises
- Pain management strategies
- Safety precautions
- Walking with crutches or walker
- Using appropriate safety precautions when transferring in and out of bed and on and off other surfaces
- Educating your family/caregiver on using safety precautions
- Improving balance to decrease the risk of falling
- Providing education to you and your family/caregiver on safety strategies and techniques (this will enable successful and safe access to home upon discharge as appropriate)
Occupational Therapy
An Occupational Therapist will assess your:
- Upper body strength and range of motion to assist during recovery
- Ability to transfer on and off the toilet and other surfaces
- Ability to dress, bathe, and perform personal hygiene
Based on this evaluation your individualized treatment plan will be developed and may focus on:
- Teaching upper body strengthening and range of motion exercises
- Pain management strategies
- Safety precautions
- Appropriate safety precautions when transferring to the toilet and other surfaces
- Educating you on the use of adaptive equipment for dressing, bathing, and other activities of daily living
- Educating you on joint protection and energy conservation techniques

- Pulmonary rehabilitation

Cardiac Rehabilitation Program
The goal of the Cardiac Rehabilitation program is to help people with heart disease recover sooner and return to full, productive lives. The facility contracts with a Medical Director, Nurse Practitioner, and Medical Specialty Pulmonologist. The patient is considered an integral part of the cardiac rehab team, along with independent primary and cardiac physicians, nurses, exercise specialists, and dieticians. Cardiac rehabilitation begins in the hospital and continues on an outpatient basis after discharge.

What to expect in Rehab:
- Assessment of current functional status
- Exercise training and other therapeutic exercise
- Education and skills training
- Nutritional assessment
- Psychosocial assessment, support, and intervention if necessary.
- Smoking cessation (If needed)
- Medication management and education
- Implementation of a home treatment program and possible follow-up
Physician Services
Riverside Health and Rehabilitation has partnered with local hospital systems to ensure continuity of care. These independent physicians also utilize nurse practitioners so when they are not here, they have licensed and certified advanced practice nurses at the facility to ensure that trained eyes are available and ready to assist in making informed healthcare decisions.
